Intake, Clinical Assessment, and Ethical Documentation

Assignment Instructions

Review the Capella multimedia Counseling Center Client Intake. Explore the intake of the five cases that are presented. Choose one of the five cases for this assignment that you will bring forward into the Unit 6 assignment (Markus, Katia, Rameen, Evan, or Roberta). Use the Intake form from the Counseling Center Client Intake media and submit it along with your written assignment.

You may use the Intake, Clinical Assessment, and Ethical Documentation Template (given in the resources) to structure your paper.

In this assignment, you will focus primarily on the introduction, opening, and beginning phases of a session that leads into the body of session where the assessment process begins.

In your paper:

  • Include a summary of the selected case scenario, including an explanation of the presenting issues. (This scenario will be used in this assignment as well as in the Unit 6 assignment.)
  • Provide an overview of the required tasks that must be accomplished in the introduction, opening, and body of a session, drawing from your Clinical Interviewing text.
  • Explore the ethical, legal, and cultural considerations in this phase of treatment, along with strategies used to accomplish these tasks. Incorporate the ACA code of ethics, applicable laws, and at least one academic research journal to consider in this case.
  • Complete the Intake form for your selected client, if you have not already done so for the Unit 3 assignment, using the Counseling Center Client Intake media piece. Using the information from the completed intake, select and apply two assessments tools (one suicide assessment and one other assessment) that are appropriate for the case scenario. Provide a rationale for your choices.
  • Document how you would discuss the importance of these assessments with your client, including your rationale for use as drawn from the intake, supporting with evidence from the literature.
  • Explore your ability to use interviewer tasks, skills, and consideration of assessments during this phase of the treatment process.
    • How do you believe you can accomplish the intake tasks?
    • How might you effectively work with this client? What strategies might you use?
    • What challenges did you encounter while working through this activity?

Submit your assignment and the completed Intake form in PDF format to the instructor in the assignment area.

Introduction

Start writing your introduction here (1–2 paragraphs).  An effective introduction prepares the reader by identifying the purpose of the paper and providing the organization of the paper. Please double-space and remember to indent all paragraphs throughout your paper (not block form!).  Aim to keep your writing objective by using 3rd person. Unless required for the specific assignment, please do not include a Table of Contents, as it is not APA style.  Review paper guidelines on page requirements and number of sources required (if provided).  Unless citing a classic work, aim to cite research articles and texts published within the past five years.  Please use headings throughout your paper that are consistent with the paper’s scoring guide (that way you ensure you are adequately addressing all required areas). Remember to double space between sentences.

When you finish writing your paper, re-read it to check for errors and make sure your ideas flow well.  A helpful tip is to read your paper aloud to yourself.  If it does not sound right to your ear—it is not working on paper!  Review your SAFEASSIGN report to check for plagiarism and opportunities to improve originality in your writing. Also, remember as a Capella learner you have FREE access through iGuide to personal tutoring services with Smarthinking.com.  Please utilize the course textbook, scholarly resources and ethical and legal codes.

Identification of Case Summarization of Skills

Identify the clinical case scenario chosen. Explain the therapeutic methods, skills, and techniques that are needed to maintain a relationship in the introductory, opening and body of a session. This includes logistics of the room, skills, and other tasks as outline in the Clinical Interviewing text and other resources.  This section should be a minimum of 3–5 sentences and should include at least one citation from your readings, applied in APA style.

Exploration of Ethical, Legal, and Cultural Considerations

In this section, specify ethical codes to consider in the flow of session from the introduction and leading into the body of a session. Next, explore specific laws that are applicable and important to consider to accomplish these tasks. Finally, explore cultural considerations when working with clients. The discussion should be a minimum of 4–5 sentences and should include citing applicable ethics, laws, and scholarly literature applied in APA style.

Assessment Identification

In this section, indicate the rationale for completing both a suicidal and other clinical assessment. Provide a rationale for the choice of the assessments and document how you will explore the importance of conducting these assessments with the specified client based upon the intake information reviewed. This discussion should be a minimum of 4-5 sentences and should include citing applicable ethics, laws, and scholarly literature on assessments in APA style.

Exploration of Interview Tasks

In this section, the counselor (i.e., you) will explore your ability to use interviewer tasks, skills and application of assessments during this phase of the treatment process. This includes indicating how you can accomplish the intake tasks, how you might work with this client, and strategies of success and potential challenges.

Conclusion

Please provide a conclusion that summarizes the main ideas of your paper.

  • Discussion Questions (DQ)

Initial responses to the DQ should address all components of the questions asked, including a minimum of one scholarly source, and be at least 250 words. Successful responses are substantive (i.e., add something new to the discussion, engage others in the discussion, well-developed idea) and include at least one scholarly source. One or two-sentence responses, simple statements of agreement or “good post,” and responses that are off-topic will not count as substantive. Substantive responses should be at least 150 words. I encourage you to incorporate the readings from the week (as applicable) into your responses.

  • Weekly Participation

Your initial responses to the mandatory DQ do not count toward participation and are graded separately. In addition to the DQ responses, you must post at least one reply to peers (or me) on three separate days, for a total of three replies. Participation posts do not require a scholarly source/citation (unless you cite someone else’s work). Part of your weekly participation includes viewing the weekly announcement and attesting to watching it in the comments. These announcements are made to ensure you understand everything that is due during the week.

  • APA Format and Writing Quality

Familiarize yourself with the APA format and practice using it correctly. It is used for most writing assignments for your degree. Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for APA paper templates, citation examples, tips, etc. Points will be deducted for poor use of APA format or absence of APA format (if required). Cite all sources of information! When in doubt, cite the source. Paraphrasing also requires a citation. I highly recommend using the APA Publication Manual, 6th edition.

  • Use of Direct Quotes

I discourage over-utilization of direct quotes in DQs and assignments at the Master’s level and deduct points accordingly. As Masters’ level students, it is important that you be able to critically analyze and interpret information from journal articles and other resources. Simply restating someone else’s words does not demonstrate an understanding of the content or critical analysis of the content. It is best to paraphrase content and cite your source. • LopesWrite Policy

For assignments that need to be submitted to Lopes Write, please be sure you have received your report and Similarity Index (SI) percentage BEFORE you do a “final submit” to me. Once you have received your report, please review it. This report will show you grammatical, punctuation, and spelling errors that can easily be fixed. Take the extra few minutes to review instead of getting counted off for these mistakes. Review your similarities. Did you forget to cite something? Did you not paraphrase well enough? Is your paper made up of someone else’s thoughts more than your own? Visit the Writing Center in the Student Success Center, under the Resources tab in Loud-cloud for tips on improving your paper and SI score.

  • Late Policy

The university’s policy on late assignments is a 10% penalty PER DAY LATE. This also applies to late DQ replies. Please communicate with me if you anticipate having to submit an assignment late. I am happy to be flexible, with advance notice. We may be able to work out an extension based on extenuating circumstances. If you do not communicate with me before submitting an assignment late, the GCU late policy will be in effect. I do not accept assignments that are two or more weeks late unless we have worked out an extension. As per policy, no assignments are accepted after the last day of class. Any assignment submitted after midnight on the last day of class will not be accepted for grading.

  • Communication

Communication is so very important. There are multiple ways to communicate with me: Questions to Instructor Forum: This is a great place to ask course content or assignment questions. If you have a question, there is a good chance one of your peers does as well. This is a public forum for the class. Individual Forum: This is a private forum to ask me questions or send me messages. This will be checked at least once every 24 hours.
