NUR 225 Incivility in Healthcare: How it Affects the Nurse and Healthcare Environment

Incivility is a rude or discourteous act. In nursing incivility and bullying are considered forms of violence and are not tolerated. The United States Department of Labor defines workplace violence as “an action (verbal, written or physical aggression) which is intended to control or cause, or is capable of causing, death or serious bodily injury to oneself or others, or damage to property” (Cybersecurity and Infrastructure Security Agency, n.d.). Bullying among co-workers, also called lateral violence, may not always be physical, but can cause emotional damage to the victim. The psychological effect this type of abuse has on the nurse can directly affect her ability to care for patients and put them at risk for unsafe events.

The healthcare metaparadigm includes person, health, environment, and nursing. The person component includes the patient as well as their spouse or other family members involved in care. The environment component includes the patient’s surroundings and how it effects their health, while the health component involves the patients’ wellness and access to health care. Finally, nursing involves safe patient care with a focus on clinical judgement to create the best patient outcomes. Incivility in the workplace can have an affect on all these components. The environment is affected, which will also have consequences for the person, health, and effective nursing.

Some hazing or bullying activities include giving a new nurse the heaviest patient assignment, giving them a hard time when they have questions, or even refusing to help them with patient care. Although harm to the patient isn’t the intent, the lack of teamwork and collaboration in lateral violence has a negative effect on the quality of patient care. It can create a negative and tense health care environment and takes the focus off the top priority, the patient. “Healthcare environments are particularly situated to foster bullies, because they are hierarchical in nature and tend to involve a great deal of change” (Zerwekh & Garneau, 2021, p. 604) Bullying is usually not a singular event, and in the nursing world typically those most likely to be targeted are new nurses, or those that appear weak and don’t speak up for themselves.

Incivility can lead to psychological problems such as post traumatic stress disorder (PTSD). PTSD is caused by trauma that can lead to flashbacks that create anxiety, as well as psychological and emotional stress in a person. Re-experiencing includes intrusive and recurrent thoughts, images, and flashbacks of the violence; avoidance and numbing include feelings of  detachment and persistent avoidance of memories that relate to the violence; and increased  arousal can include insomnia, difficulty focusing, and hyper-vigilance (Giddens, 2017, p. 369).  Avoidance, anxiety, and insomnia along with difficulty focusing can cause a disruption in the critical thinking involved with patient care. The nurse may find it difficult to process information, notice changes in a patients’ condition or may have a lack of necessary resources to  provide care due to avoiding certain situations and co-workers.
