Purpose: This paper asks you to assimilate concepts from the entire course as you
describe and assess a client system and plan an intervention that is 1) based on the specific needs of your client/client system, 2) relevant research evidence and practice theory, 3) the resources and constraints of your organizational setting, 4) your own developing skills in engaging clients and implementing interventions. When possible, your paper should be based on a real client from your field site. Please use a pseudonym for your client and disguise any obvious identifying information. If you do not have a client system to write about, please see me to discuss alternatives.
Remember: You do not have to actually implement this plan with your client system. However, your plan should be realistic given the unique challenges and opportunities of your field site. Due Mon. Dec. 9 via Turnitin link. The assignment consists of 3 sections: Assessment, Intervention Plan, and Reflection. A) Brief Assessment: Description of the client system. Address the person/s AND their environment as well as your setting of practice. Readings from Weeks 6 & 7 should guide your assessment. If you turn in an assessment you have already written for your field placement, it is not likely to reflect the perspectives we have addressed in this class. Turning in work from another class is considered a violation of academic integrity at SU. 1.Basic information (e.g., age, gender, race/ethnicity, etc. as relevant to the case) 2.Presenting problem, client’s readiness for change 3.Background history of the problem and client/system strengths 4.If you will use a standardized assessment tool, describe your findings; if not, identify an assessment tool that might be helpful. Where relevant, you may also wish to make some tentative hypotheses about psychiatric diagnoses. B) Intervention Plan: 1.Statement of goals. Address long and short-term goals. Please revisit SWK 601 material on goal setting. Your goals should be relevant to your assessment, attainable, measurable, and developed in collaboration with your client. 2.Intervention plan. Describe in detail what you will do to help your client system attain the goals you have developed together. You should explain how you selected a particular intervention approach (e.g., CBT, narrative family therapy, interpersonal psychotherapy, assertive community treatment, etc.). Hint of the century: Readings from Week 3 and Week 8 are essential to addressing this part of the assignment. 3.Intervention strategies with interdisciplinary teams. What referrals, collaborations, or linkages would you establish as a part of your intervention plan? Identify additional services that your client system may need and describe what specifically you will do to connect them with those services. (Never assume that handing your client a phone number means that they will make contact with a provider. Does your client have a phone/internet access? How about transportation? Childcare? Time?) 4.Brief statement on evaluation. Please briefly describe how you will know whether your intervention is working. Remember that this should reflect the “golden thread” of your assessment, goals, and treatment plan. If you are using a standardized assessment, consider whether the same instrument might be useful in evaluating your intervention. C) Reflection: 1.What is likely to be challenging about implementing this plan? What seems promising or exciting about it? What are your concerns or worries as you imagine undertaking this plan? 2.What knowledge and/or experience prepares you to implement this plan? 3.What additional training or knowledge do you need to seek out in order to help this client system effectively? How would you get it?
4.How feasible is the plan given the realities of your setting of practice, sources of funding, and community resources? 5.Given the specifics of this case and of your own personal history, how will you guard against compassion fatigue or other negative effects on yourself as a social worker?
