Case Study

Overview

Case Study/Familia Gonzalez: Ana Gonzalez is a 36-year-old Latina, married to Adrian, 46, and mother of two sons (Allan, 8, and Arthur, 13). She recently moved from Miami to Chicago due to her husband's job relocation. Her husband Adrian's parents (Antonio and Asuncion, a Cuban immigrant couple, 70 and 72 years old, respectively) live in this city, with whom Ana maintains a good relationship. Ana was a housewife, she used to work as a clerk in a local pharmacy while finishing college, but since Allan was born she stopped working to dedicate herself entirely to housework. In Chicago she has begun to look for part-time work to help with the family finances, taking into account that her children are already in school, and that the youngest of them, Allan, is assimilating the change well. Ana emphasizes Allan because in her opinion he has been the most problematic of the brothers. She says that unlike her brother Arthur, Allan has had to live through very complicated family stress situations since birth, starting with the postpartum depression she was diagnosed with, which from her point of view prevented her from creating emotional ties with Allan. Ana explains that the postpartum depression lasted more than a year, during which she barely had contact with the youngest of the children, and for that reason Allan avoided having any contact with her since he was little, even after his mental health improved. Ana adds that she currently feels isolated, has limited social connections in the new city and expresses feelings of sadness, anxiety and occasional irritability. Ana reports that her relationship with her husband has been strained since the move and they frequently argue about finances, parenting and her desire to return to school to obtain a degree in early childhood education. She says that the main arguments she has with her husband Adrian are about the way he raises the children, reproaching her for being too permissive with the children, for challenging his authority, removing the punishments he has imposed on them and letting them act without setting limits. Ana admits that she is an overprotective mother and is very bothered by Adrian's "dictatorial" approach with both children, forcing them to obey him blindly, setting rigid boundaries, allowing one-way communication: "Things are because he says so and that's it." Ana is also concerned because she believes that Arthur, the oldest son, is gay and comments that this would be a very stressful process for the entire family, raised under the Christian faith and with the most sexist father in the world. Ana reports that she is not sure of her teenage son's sexual orientation, but that a mother knows these things. Ana claims that he is having trouble sleeping, difficulty concentrating, and has lost interest in some of his usual hobbies. Ana has no medical conditions, nor a history of substance abuse; she rarely consumes alcohol and does not smoke. The patient has been hesitant to seek professional help because mental health is somewhat stigmatized in her cultural context, but she has noticed that her mood is affecting her family dynamics and wants to address it.

1- Prepare a biopsychosocial assessment of this patient based on the Generalist Intervention Model or Helping Process (Do not use any pre-established templates found in your workplace or on the Internet). Focus on describing the three fundamental aspects of a biopsychosocial assessment and do not forget to add the spiritual part. 2- Research about the 7 stages of the Generalist Intervention Model/Helping Process. Briefly describe each of them, their importance within Social Work and explain how you would use them in the case described above.
