Overview
Special education teachers must be skilled in explaining and guiding the transition planning process for the IEP team. The ability to clearly articulate the necessary components of the transition plan and help the group create and implement a plan using person-centered planning strategies and the goals, needs, and desires of the student is a crucial skill in successful professional practice.
Part 1: Transition Planning FAQs
Until they reach high school, some students with exceptionalities and their parents/guardians may not have had a thorough explanation of the transition planning process and the various stakeholders/roles associated with the completion of a transition plan. - Purpose of the transition plan
- Explanation of how the transition plan is encompassed as part of the IEP
- Transition goals and assessments
- Credits and course of study plan
- Age of majority
- Activities aligned to transition goals
- Sample high school transition meeting agenda
- Social, emotional, and educational well-being considerations
- Community resources
Support the FAQ document with 2-3 scholarly resources.
