Biology Question

Overview

Introduction: The local and state public health systems are not sufficiently funded to do this work on their own. Community partners are often a part of the discussion for both the community health assessment (CHA) and the community health improvement plan (CHIP). Finding community partners that share their time, employees, resources, expertise, and funding is essential to creating a sustainable change in a community to address the selected health issue.

Assignment: In your portfolio assignment, you should have assessed the demographic profile of your community. Next, you should have evaluated the high-risk public health areas. Lastly, you should have a method of prevention for each tier of prevention with an estimated impact. For the last part of your project, identify 10 community partners that have a history of volunteering, focus on a specific issue within the community, or generally engage in outreach. These can include but are not limited to non-government organizations, private organizations, elected officials, universities, community colleges, k-12 education, and local governmental offices. Examples of participants can often be found in the list of participants in the community health improvement plan or the community health needs assessment for your county or state. Please provide comprehensive contact information including address, phone number, e-mail, and website (if available).

It is strongly recommended that you submit all assignments to the TurnItIn Originality Check prior to submitting the assignment to your instructor for grading.
