Formative 1:
You have learned about population health across the continuum of care and how government driven value based initiatives support healthcare organizations to achieve higher quality of care, lower cost of services, and better access to patients. In this assessment, we will take a closer look at CMS’s Accountable Care Organization model and the Care Transformation Toolkit, outlining an ACO strategy for care delivery.
Take a look at CMS’s Innovation Model material that guides health systems in moving toward a value based system. These research and innovation programs allow health organizations to take risks with testing new methods of care delivery. Read more about CMS’s Innovation ModelsLinks to an external site.. Specifically pull up the Care Transformation Toolkit Links to an external site.. This toolkit is part of an ACO strategy for development and implementation of programs that transform delivery of care.
Write about how programs such as telehealth and home health provide high risk patients with alternative access to healthcare. Use the following questions as prompts for your assessment.
- How does CMS support these programs through value based initiatives?
- What effect does this program have on the Triple Aim (quality, cost and access)?
- How does this program affect the various levels of care across the continuum (acute, ambulatory, long term)?
- What pros and cons do you see as you learn about this program against today’s healthcare challenges?

Formative 2:
Healthcare is faced with many challenges related to various levels of care and how it connects to the hospital, ambulatory services and long-term care. Healthcare organizations must look into convenient care options and other patient services to drive more patient access to healthcare. Essentially a continuum of care is a system that provides a comprehensive range of health services, so that care can evolve with the patient over time. With the understanding that a patient's health may be most vulnerable during gaps in care, the continuum of care exists to ensure those gaps are filled. A continuum of care results in better care that has less stress, is more convenient, is more social, is easier on the patient and their families, and allows patients to age in place for as long as possible.
Provide a visual of various levels of care across the continuum. Use the scenario below.
Scenario:
- Consider the patient’s experience after a joint replacement surgery.
- Show different levels of care and how a patient going through knee surgery start with the primary care doctor, then goes to the hospital to have the actual surgery, then goes to physical therapy (Ambulatory care specialist) for post-surgery (post- acute) care and then back to the primary care doctor for routine follow up.
- Must include connection to primary care, hospital, ambulatory care specialty (physical therapy), and back to primary care.

Summative:
Healthcare is going through an array of continuous changes. Current issues around quality, access and cost have been challenged at a new level with COVID. Healthcare employee burnout rates are higher than ever. And the need for streamlined collaboration and communication for a value driven future is key on the priority list.
In this assessment, you will have the opportunity to outline how these healthcare challenges are intertwined with reimbursement structures to improve value to the patient.
- Use your drawing from the formative assessment. Add healthcare challenges and how each level of care is affected by those challenges.
- Use the list of health challenges from the formative assessments. Consider how these health challenges affect the different levels of care.
- List the individual healthcare challenge next to the level of care it would affect.
- For example, if one of the challenges is that a patient with knee replacement surgery is returning to the hospital within 30 days of discharge, which level of care is affected by this?
