Imagine you are an Office Manager for a large medical practice. You have been asked to design a new patient intake form for the practice. Apply the best practices from Chapter 4 and sketch out a 2 page form for your practice. (Create in Word and upload.)
In the Discussion Board answer the following questions:
- Why are paper forms considered as source documents?
- Why is it so important to properly develop data collection tools?
- What data is collected and how is it used by the medical practice?
