Overview
this question have 2 parts wich are, the FIRST part (1000 words) describing the elements failed in the incident (process safety leader ship, safety culture, procedures, training, and design) , the SECOND (750 words) part about leading and lagging metrics for the incident. more details will be provided later. I uploaded the investigation report which have all the information you need for part 1 & 2, and I uploaded and book for different leading and lagging metrics, and I will provide extra information.
