Overview
Week 2
For this assignment, you will write a 3- to 5-page paper detailing the information you found from your research regarding your chosen measure (e.g., sepsis care or colonoscopy follow-up) or at the team’s selected hospital. Recall from the Project Brief: Quality Improvement Initiatives that your team is to select a hospital from the Medicare.gov https://www.medicare.gov/care-compare/?providerType=Hospital webpage.
In order to fulfill the CEO’s request to identify the low-performing quality and patient safety issues in the hospital, browse through the following categories listed on the webpage for your team’s selected hospital:
- Patient Survey Rating
- Timely and Effective Care
- Complications & Deaths
- Unplanned Hospital Visits
- Maternal Health
- Psychiatric Unit Services
- Payment & Value of Care
- After you select a hospital, explore the categories listed on your chosen hospital’s webpage to understand how it scores. Be sure to scroll down and expand each category to review the measures nested within the category. You can refer to the following examples:
- Example 1: The ABC Hospital, under the category “Timely and Effective Care" for Sepsis Care, scored 53% for this measure. This number is the percentage of patients who received appropriate care for severe sepsis and septic shock. The national average was 60%, and the state average was 55%. This is a low-performing quality and patient safety issue that could be explored.
- Example 2: Under the category “Timely and Effective Care” for Emergency Department Care, this hospital had 146 minutes for the measure “average (median) time patients spent in the emergency department before leaving from the visit.” The national average was 172 minutes, and the state average was 158 minutes. But another hospital in the same area had an average of 125 minutes. This measure could be explored as well.
- Describe your chosen hospital’s services or products and customers served.
- Evaluate one measure of your choice.
- Compare your chosen hospital’s performance regarding your selected measure with the state average or national average.
- Remember that each team member must have a unique measure.
- Discuss the importance of your selected measure (e.g., accreditation status, patient safety, and/or hospital financial status).
- Define two to three SMART goals that will improve your selected measure.
- Be sure to include all five elements—specific, measurable, attainable, relevant, and time-bound—in each goal.
- The following are examples of appropriate goals:
- To implement an up-and-running emergency department tracking system by 12/31/2024.
- To hire and train an additional five nurses to meet the staff-to-patient ratio during peak times by 12/31/2024.
- To reduce the turnaround time at 10% for lactic acid/lactate levels laboratory testing by 12/31/2024.
- Analyze specific local, state, or national policies (e.g., The Joint Commission standards) that have been developed to improve your selected measure based on evidence-based practice research.
- must be 3 to 5 double-spaced pages in length (not including title and references pages) and formatted according to APA style
- must include a separate title page with the following in title case:
- title of paper in bold font
- Space should appear between the title and the rest of the information on the title page.
- student’s name
- name of institution (The University of Arizona Global Campus)
- course name and number
- instructor’s name
- due date
- title of paper in bold font
- must utilize academic voice.
Week 3 - Each quality improvement (QI) initiative is composed of systematic inputs, processes, and activities. Additionally, the QI initiative is a team process involving various stakeholders. Whether the initiative seeks to improve wait time, diabetes care, or other important goals, a team effort can help achieve significant and lasting improvements. Without proper resources as inputs, the QI activities might not reach their optimal outcomes. In your paper,
- Identify at least three quality improvement or risk management tools or methodologies (e.g., flowchart, Six Sigma, etc.) needed for your QI initiative.
- Be sure to provide the rationales for your selected tools or methodologies.
- Determine the stakeholders involved in accomplishing your QI initiative.
- Be sure to provide the rationales.
- Examine other resources (e.g., time, finances, materials, etc.) required for your QI initiative.
- Be sure to provide the rationales.
- must be 3 to 5 double-spaced pages in length (not including title and references pages) and formatted according to APA style

Week 5 - Identify at least three quality improvement or risk management tools or methodologies (e.g., flowchart, Six Sigma, etc.) needed for your QI initiative.
- Healthcare organizations face constant changes in their internal and external environments. Continuous quality improvement (QI) has no end and depends on effective teamwork to implement successful QI initiatives to meet accreditation requirements and exceed patients’ expectations. For this last component of the QI initiatives project, your designated team leader must compile your team’s work for the hospital’s quarterly meeting. The team leader must collect an individual QI initiative slide from each team member on Day 1. The team leader must use the Collaborative Project: Quality Improvement Initiatives PowerPoint Presentation Template and include the team composition on one slide and the mission and vision statement on another slide, followed by each team member’s presentation. The team leader will share the final PowerPoint presentation with team members before the due date, and each team member must individually submit the Collaborative Project: Quality Improvement Initiatives assignment to their individual student Dropbox. In your group presentation,
- Summarize the team composition.
- Be sure to list the names and job titles in the QI department.
- Identify the QI department’s mission and vision statement.
- Organize the QI Initiative for each team member.
- Be sure to include the member’s name, QI Initiative, and SMART goals in each slide.
- Summarize the team composition.

Week 6
A successful quality improvement (QI) initiative is essential for helping a healthcare organization boost efficiency and improve its business model. It is critically important that the healthcare organization measures and monitors whether the anticipated outcomes are achieved. Health information technology (HIT) is often used to support systematic QI efforts by providing timely and valuable feedback on performance.
For your Quality Improvement Initiative final paper, you must complete an 8- to 12-page paper that details your QI initiative.
Requirements
In your project,
- Summarize your Quality Improvement Initiative: Part 1 and Part 2.
- Assess an appropriate advanced HIT that can be used to support your QI initiative.
- Be sure to provide the rationales for your selection.
- Describe how the organization can incorporate your QI initiative into their overall strategic plan.
- Determine how to evaluate the effectiveness of your QI initiative.
