Overview
Disease management effectively requires an understanding of the many resources for disease management, how behaviors connect to health, what patient education is and sensitivities to cultural needs. Your first clinical application essay will focus on one of the subjects covered this week. This assignment will be a 500 word essay demonstrating an understanding of the course module concept presented in week one through practical application. Choose only one of the following below.
1.What steps should be taken to develop a comprehensive disease management program and why?
2.Identify a Health Behavior Model and describe how you would apply its’ principles in educating a non-compliant asthmatic patient to improve their self-management.
3.Using either the APIE or ASSURE method for patient education, construct an education plan for a COPD patient and identify 4 problems that could impact the quality of your interview.
4.Essays will have at the minimum three paragraphs; Introduction, Body and Closing.
