Overview
Optimum management in chronic disease requires vigilance to maintain best practice processes. In this module we covered three subjects that highlight practice change and developing value-added services. This assignment will be a 500 word essay on one of the following demonstrating an understanding of module concepts through practical application:
1.Identify a chronic disease management service of your choice and apply the principles covered in this module to make recommendations for change. Answers are to include what service change is recommended, why it is important, steps for change, and how would you evaluate the impact of the change.
2.Identify and describe the components and necessary steps in the discharge planning for a chronic pulmonary patient.
Process: Module lectures and required resources to answer the prompts. External resources require citation. Essays will have at the minimum three paragraphs; Introduction, Body and Closing.
