THE DILEMMA
Consider the following scenario. You work for a hospital and one of your largest commercial insurers comes to a round of negotiating. They want to move away from a fee for service model to a value-based model for their members. You need to decide your recommendation: will you do it?

Step 1: List as many factors as you can that need to be taken into account in the decision. A table has been included at the end of this document for this purpose.
Step 2: Push yourself to think of more factors to be taken into account. Consider the implications - both positive and negative. Consider why the insurer is coming to the hospital asking for this change in payment arrangement.
Step 3: Push yourself even further to think of more factors to be taken into account. What further analysis or information might come to light with further research that would be important for this decision? What events might happen in the future that could impact the situation? For example, what if the insurance company decides to form a narrow network without you because of the fee for service model?
Step 4: Take a look at your list. Circle the five most important things to consider when making such a decision?
Step 5: Based on your list, and your ideas about which of these considerations is most important, write [or video record] your argument to advocate with administration for a decision to move to a value-based model or not.
