AFE Community Experience: Site Approval

Overview

Site Approval Form Download Site Approval Form: Due by the end of Week 3

1. Research your options for a suitable site.
2. Secure your chosen site.
3. Submit this Site Approval form for approval of the chosen site by the end of week 3.
4. If you plan to complete your hours at multiple sites, add the locations to the same form and resubmit for approval.

Community Experience Hours:

1. After your site(s) and plan(s) are approved, you may complete the 22.5 Community Experience hours.
2. After each experience, record your hours on the Project Reporting Log, Validation of Hours, and Journal Entries Download Project Reporting Log, Validation of Hours, and Journal Entriesform.
3. Create a journal entry at the bottom of the same form for each experience.
4. Continue this process until you have completed all hours.
   • You will submit your Project Reporting Log, Validation of Hours, and Journal Entries Download Project Reporting Log, Validation of Hours, and Journal Entriesby the end of Week 10. You do not need to submit your logs and entries each week.

End of Week 10 Tasks:

1. You have now completed the 22.5 AFE hours.
2. The Site Coordinator will validate your hours using a pen during each visit.
3. Submit the completed and validated 'Project Reporting Log, Validation of Hours, and Journal Entries Download Project Reporting Log, Validation of Hours, and Journal Entries' form to your evaluator for grading. Ensure it contains 22.5 documented AFE hours.
4. If you cannot complete these required hours before the end of week 10, you will be assigned an 'Incomplete, Remediation' (IR) status.
