Overview
Week 6 Practice Cases
Topic: Cardiovascular and Respiratory Systems (CPT 2026)
Cardiovascular System Cases
Case 1: A 68-year-old patient with chest pain undergoes a left heart catheterization with selective coronary angiography. No intervention is performed. Mild coronary artery disease is found.
Case 2: During cardiac catheterization, a significant lesion is identified in one major coronary artery. A drug-eluting stent is successfully placed in that single coronary artery.
Case 3: A 75-year-old patient with symptomatic bradycardia undergoes insertion of a dual-chamber permanent pacemaker, including placement of right atrial and right ventricular leads and pulse generator.
Case 4: A patient undergoes a complete transthoracic echocardiogram, including 2D imaging, M-mode recording when performed, spectral Doppler, and color flow Doppler.
Case 5: A patient with peripheral arterial disease undergoes percutaneous transluminal balloon angioplasty of the left superficial femoral artery. No stent is placed.
Respiratory System Cases
Case 6: A pulmonologist performs a diagnostic flexible bronchoscopy for evaluation of chronic cough. No biopsy or additional procedure is performed.
Case 7: During bronchoscopy, a bronchial lesion is identified and a biopsy is obtained using forceps.
Case 8: A patient with pleural effusion undergoes thoracentesis without imaging guidance.
Case 9: A patient presents with pneumothorax and a chest tube is inserted for treatment.
Case 10: A patient with lung cancer undergoes an open right upper lobectomy.
For each case, assign the appropriate CPT 2026 code(s) and determine whether any services are bundled according to CPT guidelines.
