BHE 310 health promotion disease prevention

You will create this assignment following the Assignment Detail instructions below:

Healthy People is an initiative by the U.S. Department of Health and Human Services (HHS) in conjunction with the Office of Disease Prevention and Health Promotion (ODPHP). Every 10 years, data are evaluated to determine health priorities in which to focus on for the next decade. This is the federal government's formative assessment process. This initiative is important because the leading health indicators (LHIs) identified through these data will drive federal spending and focus for the next several years. Aligning federal, state, and local priorities helps create shared focus and resources, allowing funding to go farther. This increases resources that benefit health at all levels. Remember, the setting, context, and cost of a program are important because these factors (e.g., personnel, resources, and delivery conditions) have important implications on outcomes and processes of change.

Choose a specific LHI from this resource: Leading Health Indicators.

Create a new proposed health intervention to target your chosen LHI. Use the following template to create an evaluation plan for your proposed intervention: Evaluation Plan Template.Submit your template, along with a Word document that describes the LHI chosen, the intervention, program goals, and evaluation plan. Be sure to clearly articulate and justify the evaluation methods you choose.
