EXCEL SHEET FORMAT

Overview

Hi there

I’d really appreciate your help in creating an Excel data collection sheet for a QIP project. I’ve outlined the columns below ideally with dropdown boxes where specified to make data entry and analysis easier.

Columns required:

1. Case number
2. Patient MRN
3. Sex → dropdown: Male / Female
4. Age
5. Admission date → format: DD/MM/YYYY

Respiratory diagnosis as headline then below it (separate columns, each with dropdown Yes/No):

6. Obstructive airway disease Yes/No

7. Obesity hypoventilation Syndrome Yes/No

8. Chest wall / neuromuscular disease Yes/No

9. Cardiogenic pulmonary oedema Yes/No

10. No data / not recorded Yes/No

11. Other Yes/No

1. Type of blood gas measurement → dropdown: Arterial / Venous / Capillary / Unknown
2. First pre-NIV pH
3. First pre-NIV PaO₂
4. First pre-NIV PaCO2
5. First pre-NIV HCO₃
6. Date of decision to start NIV
7. Time of decision to start NIV
8. Date NIV commenced
9. Time NIV commenced
10. NIV commenced within 60min Yes/No
11. Place where NIV was initiated → dropdown: ICU / ED / Respiratory ward / General medical ward / No data / not recorded
12. Was there a delay in starting NIV? → dropdown: Yes / No / Do not know
13. If yes, reason for delay → dropdown: Failure to recognise need for NIV, Lack of equipment, Patient required transfer to another area, Lack of beds, Other
