Health & Medical Question

Overview

Digestive System

Instructions: Assign the correct CPT code(s). Apply CPT 2026 guidelines. Pay attention to approach, extent, and interventions.

Case 1 A 58-year-old patient undergoes a colonoscopy due to a positive FIT test. The colonoscope is advanced to the cecum. During the procedure, a 1.2 cm polyp is identified in the ascending colon and removed using a snare technique. No other lesions noted.

Case 2 Patient presents with melena. An EGD is performed. The physician advances the scope through the esophagus, stomach, and duodenum. An actively bleeding gastric ulcer is identified, and hemostasis is achieved using thermal coagulation. No biopsy performed.

Case 3 A screening colonoscopy is initiated; however, due to poor bowel preparation, the physician is only able to advance the scope to the transverse colon. No interventions performed. Procedure discontinued.

Case 4 Flexible sigmoidoscopy was performed due to rectal bleeding. A lesion is identified in the rectum, and a biopsy is obtained using forceps. No additional procedures performed.

Case 5 Patient undergoes a laparoscopic cholecystectomy. During the procedure, an intraoperative cholangiography is performed to evaluate the bile ducts. The gallbladder is removed without complication.

Urinary System

Instructions: Assign the correct CPT code(s). Follow CPT 2026 guidelines and consider bundling rules.

Case 1 A patient undergoes cystourethroscopy for evaluation of hematuria. During the procedure, a bladder tumor is identified and removed using fulguration.

Case 2 Cystoscopy is performed. A ureteral stent is inserted into the left ureter due to obstruction. No additional procedures performed.

Case 3 A patient undergoes transurethral resection of the prostate (TURP) for benign prostatic hyperplasia with urinary obstruction.

Case 4 Patient presents with kidney obstruction. A percutaneous nephrostomy tube is placed under imaging guidance for drainage.

Case 5 Cystoscopy is performed with litholapaxy for the removal of a bladder stone measuring 3 cm. The stone is successfully fragmented and removed.
