In this discussion board assignment, you will read the following case study. Decide upon the appropriate interventions. Analyze the patient's presentation and symptoms. Discuss the appropriateaction that needs to be taken at this time along with the possible outcomes. Use evidence-based methods to guide your treatment plan. What type of pulmonary disorder may this be indicative of? What type of neonatal cardiac disorders may present with hypoxemia?
In addition, include any experience you may have had in treating this type of patient situation.
Case Study: A 900-g baby boy is born by spontaneous vaginal delivery at 26 gestation. The Agar scores are 4 and 6. The baby is active but grunting. PPV is initiated to maintain respirations. After 5 minutes, the baby is placed on nasal CPAP with a pressure of 6, and 70% FIO2. An x-ray and an abg is ordered.
X-ray shows a ground-glass pattern and after 30minutes, the abg is as follows:
pH- 7.12
PCO2- 80
PO2- 50
Step-by-Step Assignment Instructions (Meets Expectations)
Part 1, Initial post - due at 11:59 PM on Wednesday of this week
- Analyze the patient's presentation and symptoms, including possible cardiac and pulmonary issues as it relates to the patient's presentation.
- Discuss the appropriate action that needs to be taken at this time (modality change, vent settings, medications) along with the possible outcomes.
- Summarize and include artifacts of evidence-based methods you used to guide your treatment plan (articles, current practice protocols, flow chart, etc..).
- Compare and contrast how the pathway of this situation would go if presented in the Emergency and Room versus a delivery unit with a NICU readily available.
