Write a 3–4 page executive summary, which includes a project plan outlining the intervention steps, a conceptual model for change, and a systematic evaluation plan that details the intervention steps, time frame for completion, the individual responsible for each step, and current metrics and expected benchmark metrics.
This assessment focuses on the potential challenges faced by advanced practice nurses (APRNs) in implementing person-centered care and strategies to address these challenges. You will also work on developing a strategic outline for a person-centered care intervention, understanding the complexities and nuances involved in practical applications within healthcare environments.

For this assessment, please complete the following:
- Develop a strategic outline for the person-centered care intervention. This should include:
- A detailed time frame.
- Specific components of the intervention.
- Clearly defined expected outcomes.
- Compare current data metrics with the targeted improvements you aim to achieve through the intervention. Identify at least four measures you are tracking to improve care.
- Illustrate a clear connection between the intervention and at least two goals of the organization's strategic plan, demonstrating how the intervention supports the overall plan to improve care.
Your assessment should also meet the following to requirements:
- Written communication: Should be free from errors that detract from the overall message.
- APA formatting: Adhere to current APA style and formatting standards. Include a title page, an introduction, a conclusion, a title above the first paragraph, and level headings to define each section of the paper.
