Discussion - My Patient Population of Practice
Overview
For this week's discussion board, you are required to introduce your peers to your Patient Population of Interest (PPoP). You need to explain the reason behind choosing this particular patient population and provide a summary of the disease management plan that would be required to overcome a challenge or support a transition. Furthermore, you need to present the available evidence that suggests there is an issue with the current patient care for your PPoP. This can include expert testimony, patient outcomes, and literature.
