Health & Medical Question

In this assignment, you will submit your outline for the Disease Management Plan. Use the outline listed below to guide the literature or concepts you want to explore. Provide a comprehensive overview of the disease management plan for the identified Patient Population of Practice (PPoP). Although the overview is thorough, the document is a living document that will evolve throughout the course. This is just an outline and will be the roadmap to draft sections. Please consider the ESLO rubric, Section IV, and the literature explored throughout the course to incorporate your annotations, articles, resources, studies, etc. Use the outline below, pair with added resources (annotations, articles, resources, studies, etc), and Zotero to begin the framework to draft your capstone paper, one step at a time. Please visit the full Disease Management Program Guide document and download it for your review.

SECTION IV

Introduction- IL 1, IL 2, IL5 (1 pages)

This segment establishes the framework and scope of your PPoP care plan. The introductory section should provide a comprehensive summary of the disease management plan for the identified Patient Population of Practice (PPoP). This context encompasses essential elements such as patient demographics, healthcare environment, required resources, expected outcomes, and the chosen means of gauging effectiveness, which may take the form of a proposed research inquiry.

Background- AL1, AP1, AP2, IL5 (1-2 pages)

This section provides background knowledge to understand the positioning of your PPoP within the healthcare landscape. Including demographic and epidemiologic data, social and economic challenges faced by the PPoP, and the healthcare system's landscape.

Literature Review- IL3, IL5 (1-2 pages)

When writing a literature review, it is crucial to provide context by introducing the topic and its history, relevance to the field, recent developments, and controversies. You should also explain the scope and objective of the review, identify the sources of information, and discuss any limitations or biases in the existing literature.

Clinical Presentation- IL5 (2-3 pages)

This section aims to give the reader a comprehensive understanding of the disease affecting your PPoP. It encompasses a detailed description of the condition's pathophysiology and the associated signs and symptoms. It is important to relate the clinical presentation to the therapy, treatment, and approach to care in an acute setting. Moreover, it is crucial to outline the conditions required for administering your disease management plan effectively for your PPoP.

Expert Testimony- AP3 (1-2 pages)

This section provides the reader with additional context from experts within the healthcare setting. You may include physicians, respiratory therapists, nurses, allied health professionals, case managers, or disease managers. In addition, in this section, you will reflect on the disciplinary perspectives accompanying and informing your disease management plan. You may utilize clinical practice guidelines, agency reports (e.g. CDC, NIH, etc.), or other disciplinary guidelines as Expert Testimony.

Disease Management Plan- AL 3, AL 4, AL 5, IL5, AP4, AP5 (3-4 pages)

This section provides the reader with recommendations that support your disease management plan. First, restate the PPoP and the plan to address a problem or issue. Then, outline your recommendations and rationale.

Recommendations- AL 6, IL5 (1/2 page)

The recommendations section of a manuscript suggests actionable steps for future research or practice. It follows the conclusion section and provides an opportunity for the author to propose new ideas. It is important to summarize the key findings and explain the relevance of the recommendations to the broader literature or field. The recommendations should be specific, actionable, and acknowledge any potential limitations.

Limitations/Implications- IL 6, AL 6 (1/4 pages)

This section gives the reader an understanding of the limitations to address in a disease management plan. Resources should come from your literature review and expert Testimony.

Conclusion- IL5, IL 7, AL 7 (1/4 page)
