Brief Overview of the Assignment
Students, in the last assignment of the course, you will be incorporating your POI/implementation plan into an Evidence Based Practice (EBP) model. The purpose of this assignment is to allow you to showcase your project ideas in a visual (EBP model) that you could actually present to the appropriate stakeholders at your facility for consideration of implementation. It is similar to an Executive Summary or Talking Points Paper that you might be asked to present giving a high level overview of the project ideas. Note that I said high level overview---your papers provided the details needed earlier so now we want to focus on the big picture. In summary, this project serves as a culmination of your work this semester with a few extra details added to the model.
Tip: if you missed something earlier on your prior assignments and you see that same topic being covered on the model, make sure you review your prior feedback to include what was missing.
Assignment Instructions:
Part 1: EBP Implementation Appendix
1. First, complete the attached EBP Implementation appendix:

2. A reference page is required for the EBP Implementation appendix.
3. Use the information from the completed appendix to create the selected EBP model in the next part of the assignment.
4. Submit the EBP Implementation Appendix document with the model.
Part 2: Model Selection
1. View the different models and select which one you think will fit your POI Implementation best. While there are many other EBP models, we are asking you to ONLY choose from this list below. Your selection should be based on which implementation process works best for your POI, facility, population, etc.
2. Accompanying each model is a rough draft of the template you may use. You are welcome to create your own as well. However, please note that I have only created a rough draft with minimal information on the template. You are expected to know what is needed to complete each section of the model selected. The templates provided are a good starting point and do not reflect a complete model. Students are welcome to change the alignment, colors, shapes of the templates to fit their needs better. However, you must include the areas of the model and additional information requested from the rubric using the EBP Implementation Appendix Template.docxDownload EBP Implementation Appendix Template.docx.
3. Models differ in required information. Example 1: -if a section of your model requires you to add the literature used to support your POI, then you should include the titles of the Guideline, Meta-analysis, single studies, etc. used in your prior assignments. Example 2 -if a section of your model asks you to identify an expected outcome then you should review your outcome from your prior assignments and add that to the model, i.e. "reduce medication errors by 20% on all units" Models naturally differ so be sure you choose the model that fits your topic and comfort level best.
