This assignment is intended to help you learn to:
- Apply knowledge and theory related to the prevention, diagnosis, and management of select uncomplicated common acute and chronic health problems for individuals across the lifespan (FNP) or adolescent through end of life (AGPCNP).
- Develop a patient education video based on a topic related to public health.
Overview of Portfolio Assignment
The portfolio assignment consists of three parts. The assignment will culminate in a patient education video centered around a public health topic. Part two is a video script to prepare for the patient education video. The final part is a video based on the completed script and literature review. Consider the audience or target demographic, including their level of health literacy, for the topic when completing the literature review and video script.
Instructions
In this part of the portfolio assignment, you will use the topic from your critical literature appraisal and create a script for your patient education presentation.

| Criteria | Ratings | Pts |
|---|---|---|
This criterion is linked to a Learning OutcomeContent Accuracy and Evidence-Base | 7 to >6.0 ptsProficientScript content is fully aligned with the evidence synthesized in the critical literature appraisal and covers all necessary public health information comprehensively.6 to >5.0 ptsAcceptableScript content is generally accurate and relates to the topic in the critical appraisal but may miss one or two key pieces of evidence-based information. 5 to >0 ptsUnacceptableScript contains factual errors, uses information not supported by the evidence in the critical appraisal, or does not address essential aspects of the public health topic. | 7 pts |
This criterion is linked to a Learning OutcomeHealth Literacy and Audience Appropriateness | 7 to >6.0 ptsProficientScript clearly identifies a specific target age range. Language is consistently clear, concise, and uses strategies (e.g., teach-back, analogies) appropriate for low health literacy levels and the chosen age group.6 to >5.0 ptsAcceptableScript identifies a target age range. Language is mostly plain and simple but occasionally uses medical jargon without clear explanation. 5 to >0 ptsUnacceptableScript does not specify a target age range or uses language that is overly complex and would not be appropriate for all levels of health literacy. | 7 pts |
This criterion is linked to a Learning OutcomeStructure and Timing | 7 to >6.0 ptsProficientScript is logically structured with a clear introduction, body, and conclusion. Dialogue is written for a video length of 5-7 minutes and maintains a fluent, engaging pace. The flow of information is easy to follow.6 to >5.0 ptsAcceptableScript has a discernible structure (intro, body, conclusion) but the pace is uneven, making some sections feel either too long or too short. 5 to >0 ptsUnacceptableScript is significantly under or over the 5 to 7-minute requirement. The structure is disorganized, or the transition between topics is disjointed. | 7 pts |
This criterion is linked to a Learning OutcomeGraphics and Visual Integration | 4 to >3.0 ptsProficientScript clearly and strategically notes all necessary graphics or visual text in a separate column or notation. Visuals are highly relevant, support the spoken dialogue, and effectively simplify complex concepts.3 to >2.0 ptsAcceptableScript includes notations for graphics/visuals, but they are sometimes generic or do not add significant instructional value to the content. 2 to >0 ptsUnacceptableScript contains no mention of graphics or visuals, or the visual descriptions are too vague to be integrated into a coherent patient education video. | 4 pts |
