As we come to the end of this course and program, it's time to reflect on the Information Literacy Skills that we have gathered throughout the journey. Specifically, we want to focus on the artifact you've drafted about your Patient Population of Practice (PPoP) and the Disease Management Plan created. Synthesizing such an artifact is not an easy task, and I am hopeful that you are proud of your accomplishments throughout the program and the work produced in this final Capstone Paper. Please take some time to reflect in this final discussion board.
