Scenario
You’ve just been hired as the quality improvement (QI) project lead at Serenity Community Hospital, a facility serving an aging population with complex health needs. The Chief Medical Officer has tasked you with leading a QI initiative to reduce post-surgical discharge delays, which currently average 9 hours beyond the expected time.
Your interdisciplinary team includes:
- Dr. Madison (Surgeon) – skeptical but data-driven
- Nurse Angela – enthusiastic and patient-centric
- Jordan (Case Manager) – overloaded with discharges
- Talia (Social Worker) – often excluded from discharge decisions
You are expected to lead this team through a structured improvement initiative.
