Utilize the Addiction Case Study Treatment Plan. Use the same client from u05a1, Case Conceptualization. Evaluate the case study in order to develop a mutually agreed-upon treatment plan with the client through effective communication.
Use the template to complete the treatment plan components. You do not write a paper for this assignment; the completed template is what you will submit.
Treatment Plan Components:
- Presenting Problem.
- Diagnostic Summary.
- Include stage of change, list of problems, and list of strengths.
- Theoretical Foundation.
- Discuss at least one theoretical approach you use with this client.
- Include citations to support your theory.
- Long Term Goals.
- Develop three long term goals.
- Short Term Goals.
