Overview
A counselor’s main objective is to help individuals served to identify goals and potential solutions to problems that have an impact on their well-being. As a counselor, you must be able to assess, diagnose, and conceptualize accurately before putting together a treatment plan. A treatment plan is essential to the healing process; your client will not have a map to improve without it. Every plan depends on reliable information, so a counselor must continually research treatment options that might benefit clients. Evidence-based treatment is critical to allowing your clients to live a healthier lifestyle.
Read one of the following case studies from The Clinical Assessment Workbook: Balancing Strengths and Differential Diagnosis:
- Case 14.2 in Ch. 14
- Case 21.1 in Ch. 21
- Case 21.7 in Ch. 21
Complete the following based on the case:
- Biopsychosocial Assessment, including your case conceptualization
- Treatment Plan
Use the forms located on the College of Social and Behavioral Sciences Resources site for the Clinical Mental Health Counseling program or you may write this using standard APA 7 formatting as long as you include ALL the sections.
Complete a Biopsychosocial Assessment and Treatment Plan based on the case using the forms located on the College of Social and Behavioral Sciences Resources site for the Clinical Mental Health Counseling program, OR write it up using standard APA 7 report formatting (ensure you hit ALL the sections). Use full sentences and appropriate grammar in these forms. Please make sure you have incorporated the specific feedback given to you by the professor on past submissions and made the adjustments suggested.
In your Biopsychosocial, include at least two clinical assessment tools/metrics you would use to help you assess this client and to measure therapeutic progress. Be sure to identify tools you believe would help support your diagnosis, justify your choices, and cite them. Put this in the ASSESSMENTS TO SUPPORT DIAGNOSIS section in the Biopsychosocial Assessment form to identify these tools.
In your Biopsychosocial, include a CASE CONCEPTUALIZATION as seen through your theoretical lens. In this section, spell out your perceptions of this client's presenting issues through your chosen theoretical lens, using scholarly references to support.
In your Biopsychosocial, include a comprehensive explanation of the DSM-5-TR symptoms you use in your diagnosis and clearly specify how this client meets/does not meet the specific criteria (this goes in the DISCUSSION section). Also in the DISCUSSION section, be sure you include a discussion of differential diagnoses that were considered and why they were ruled out, using the specific criteria for those disorders. For these sections, refer to the suggestions outlined in the Treatment Plan Bootcamp.
For your Treatment Plan, research common treatment goals for the chosen diagnosis before completing your Treatment Plan form. In addition to best-practice interventions, be sure to include parenting and support resources (cite them), as well as a discussion about the impact of ADHD on the family. Justify your treatment plan and all discussions and assertions with peer-reviewed research. Follow the same format we previously outlined in the Treatment Plan Bootcamp.
For Your Treatment Plan, include:
- At least two specific presenting problems. Be succinct and write in complete sentences.
- At least two therapeutic GOALS. Ensure these are written in SMART format.
- For each GOAL, include at least two OBJECTIVES (what the client will do). Ensure these are written in SMART format, and please substantiate these with citations.
- For each GOAL, include at least two INTERVENTIONS (what you, as the counselor, will do). Please substantiate these with citations. (Note: If you include a referral to an outside professional for medication management, psych eval, or a medical rule-out with a doctor, those do not count as one of the at least two interventions; we want to see how well you understand your options as a counselor treating a client with this presentation.)
Format your Biopsychosocial, Treatment Plan, and Reference page(s) according to APA 7 guidelines. Use scholarly language and voice, and proper grammar. You may combine all three into one document.
