- Due date: week 4

- Student performance evaluation criteria and procedures: you will be evaluated based upon your clinical decision making and writing skills according to the rubric not your level of self- disclosure in the self-reflection.
Description: Your supervisor believes the current intake process is faulty and ask you to revise it. Therefore, you are asked to provide a template of how to complete the intake process and a sample intake assessment for the practice. Consider reviewing the assignment rubric to better understand how your assignment will be evaluated by your instructor.
Process: For this assignment you will be asked to turn in one complete document with three parts. See below details of each part to turn in.
Part 1: TheIntake process template:
- Create a template to guide the intake process. This is the part of the training manual you would read if you were hired and needed to learn how to do a intake assessment at ECWS,INC. In the template include the following:
- Introduction: this section should describe how the intake process will flow from start to finish. How flexibility is provided. Keep in mind other tips that you may receive in the live session and Asych materials about intake.
- Outline of Pre- Paperwork: if the client is to fill out any paperwork prior to the intake session. List out in bullet point form what the paperwork would be and how the client would complete the forms. You do not need to provide a copy of any pre-paperwork forms.
- The guide for initial session with client: Keep in mind this should read as if you are instructing the counselor on how to complete the intake assessment with the client.
- Start with instruction on how the clinician can use the guide. You can provide suggestions for how the clinician can ask questions in the intake assessment, if they need to complete certain sections with specific details or follow up questions, use other methods of gathering information, etc.)
- Make sure to include a section for all intake domains from the templte provided, and any other additional sections that align with your personal style, setting, theoretical orientation, etc.
- Consider if you were to fill out your own intake assessment. (do not actually fill out the intake assessment just review and consider what it would be like to complete this as a client)
- In your 1-page reflection consider writing up the following: reactions to specific sections/questions, response styles, and potential bias. Additionally reflect on what it was like to complete the overall assignment.
