XYZ Hospital has a serious data quality problem. The organization has an outdated information system and has had a significant number of turnovers in the HIM director role, among others. Morale is low with staff—not just in HIM, but across the hospital. The organization’s information systems have unique identifiers and passwords so there are not any issues with authorship; however, they have dictation errors, as well as copying and pasting errors. They also have problems with versioning in that all versions are available for all users, and there are hundreds of duplicate health records in the master patient index. A new HIM director has been hired and her number-one priority is to get these issues resolved.
