Program Selection (Introduction)

  1. Program Requirements:
    • Must have a set start and end date.
    • Cannot be a team; it must be a sport-based program.
    • Must be an existing sport program for which you can provide a link or detailed information.

Part A: Mission and Vision

  1. Mission Statement:
    • Create a mission statement for the program.
    • Do not use or copy existing mission statements.
  2. Vision Statement:
    • Create a vision statement for the program.
    • Similarly, ensure it is original and distinct from any existing statements.
  3. Rationale:
    • Explain why you chose the specific wording for both statements.
    • Refer to lecture materials to clarify how your statements align with the requirements of a proper mission and vision.

Part B: Evaluations

  1. Formative Evaluation:
    • Develop evaluation tools/questions for assessing the program during its progression.
    • Create separate evaluations for each role (e.g., instructor, owner, parent, participant, volunteer, etc.).
  2. Summative Evaluation:
    • Develop evaluation tools/questions for assessing the program after its completion.
    • Again, include distinct evaluations tailored to each role involved in the program.
  3. Linking Questions to Program:
    • Ensure your evaluation questions are specific, relevant, and clearly tied to the program's components.
    • Avoid assuming that the person completing the evaluation has your level of knowledge (e.g., use accessible language).

Submission Guidelines

  1. Format:
    • The format is flexible (e.g., Word document, PowerPoint, or Excel), but all content must be compiled into a single file for submission.
    • If submitting in Excel, ensure tabs are clearly labeled.
  2. Order of Content:
    • Title Page
    • Introduction: Description of the program, link, and explanation of its validity.
    • Part A: Mission and vision statements with explanations.
    • Part B:
      • All formative evaluations (grouped together).
      • All summative evaluations (grouped together).
