Social Work Intervention (SOCI-317-OL-17012)

Overview

Written Assignment 2: Interviewing and Completing a Social History

Using the guidelines in the textbook Chapter 16 pages 298 - 309, interview a friend, classmate or family member as a "practice client" to complete a social history for this assignment, including Impressions and Recommendations. The interview should be in-person or video chat if all possible (not email). Phone call is acceptable if absolutely necessary. Do not interview anyone under 18 years of age.

Make sure to let “practice clients” know they are not receiving a real assessment and know all is confidential.

  1. Change the name and identifying information to keep the person's privacy, and be respectful if the person does not want to share some information. 9 points can be earned here: Share what you told the client about confidentiality, about how you conducted your interview, your client, and cover all areas of sections 1-3 of the social history, as listed in the textbook. Please use the text book example or points will be deducted.
  2. After the social history, on your own, compose a Client Goals section, including at least 3 hypothetical goals which may be appropriate for the client (assuming the client would agree) and how they could be measured. Typically, you would do this with the client but in this case, students will propose possible goals separate from the "client." 3 points
  3. Finally, share any strengths or challenges which may be present for the client to meet the 5 goals. What would you focus on as the case manager to assist this client. What would you monitor and what, if any, referrals would you make? 2 points
  4. Summarize your learning from this experience as it relates to interviewing and completing a social history. 1 point

Make sure to include at the beginning of the social history a few sentences capturing what you said to the interviewee to explain confidentiality.

*Attached is an example (draft) of a Social History with my notes at the end. This is meant to help you get started. Make sure to include a part of what you told your interviewee about confidentiality.

I am also attaching some key pages of the chapter in case you no longer have access to Chapter 16 of the book.

This also includes another example of a Social History.
