Developing a Sustainability Plan for Your DNP QI Project

📌 Purpose

Even the most well-designed quality improvement (QI) initiatives can fail if they are not planned with sustainability in mind. As a DNP-prepared nurse leader, you must be able to evaluate factors that influence implementation success, anticipate threats to sustainability, and develop strategies to embed changes into clinical practice over time.

This assignment will guide you in creating a sustainability plan that builds on your project’s outcomes, addresses organizational realities, and aligns with national quality priorities and healthcare leadership expectations.

📖 Instructions

Part 1: QI Project Recap (½ page)

  • Restate your PICOT question and briefly summarize your QI intervention.
  • Identify the setting and population impacted by the project.
  • Outline the primary outcome(s) achieved or expected.

Part 2: Barriers and Facilitators Analysis (1 page)

  • Identify at least three key factors (barriers or facilitators) that could affect long-term success.
  • Explain each factor’s impact on implementation and sustainability.
  • Use an implementation framework (e.g., CFIR, RE-AIM, NHS Sustainability Model, IHI Framework for Spread) to categorize and support your analysis.

Part 3: Sustainability Plan (1–2 pages)

Develop a practical plan to sustain your QI intervention over time, addressing the following:

  • Ongoing outcome monitoring: How will you track success post-implementation?
  • Embedding into workflow or policy: How will the change become a standard practice?
  • Staff education and onboarding: How will new staff be trained on the change?
  • Leadership support: What role will leadership play in accountability and visibility?
  • Resources and ownership: What human, technological, or financial resources are needed? Who will be responsible?

Part 4: Alignment with Organizational & National Quality Priorities (½ page)

  • Describe how your sustainability plan aligns with:
    • Organizational goals (e.g., safety, efficiency, satisfaction)
    • National benchmarks and initiatives (e.g., AHRQ, CMS, IHI, Joint Commission)
