Overview
Hello so I have attachment a teaching tool and (care plan with information inside to use for the teaching tool). Also here is additional info on the patient below. I also attached an example teaching tool in case you needed help. please reach out if you need any more info.
Pt is a 5yr old male alert and oriented x2. Pt has a hx of Hirschsprung disease. B/p 90/60, 100, 25, 98.7. Pt has oral aversion with minimal oral intake noted and failure to thrive. Ostomy noted to RUQ with large amount of liquid stool. Stoma site is beefy red and moist, no s/s of infection noted at this time. Ostomy bag emptied and changed. Positive bowel sounds in 4 quadrants with guarding to RUQ area noted. Pt rates abdominal pain 5/10 on FACES. B/L Lung sounds CTA. Tylenol administered with effectiveness. Positive ROM, able to ambulate. Pt is being monitored for abdominal pain.
