New Models of Care

Purpose: To evaluate how a new model of care fits into an existing organization.

Instructions: Select a new model of care (case management). Prepare an executive summary for your hospital leadership team on the advantages to funding this new position/role.

Include in your 3-5 page executive summary:

• Select and detail a new model of care delivery
  • Insert this into your paper.
    • Position this new role/model in your infographics
    • Summarize the potential for this new model of care
    • Explain the role of this person/position
    • Detail the benefits of adoption of this new model to the patient, hospital, and other providers of care such as social work, radiology, laboratory services, physical therapy, nursing.
• Create an infographic showing the various entities involved in the care of a patient and in maintaining a healthy community.

The paper should be 3-5 pages (not including title or reference lists), formatted based on APA guidelines, and should include in-text citations and a reference page from course resources or additional sources found.

Utilizing New Telehealth Technology

Propose incorporating telehealth into healthcare delivery services

You have been asked to prepare an executive summary for the organization's leadership team.

• Select a new telehealth technology that is of interest to you. Research both professional, business, and information technology publications and reviews.
• Select a population/ healthcare delivery site of interest (rural facilities, home health agencies, mental health clinics, public health clinics, urban health centers, teaching hospitals, safety-net hospitals, etc.)
• Propose how the use of this new technology would benefit the patients, providers, and organization.
• Evaluate what infrastructure would be needed to implement this technology, including information technology, human resources, and end-user needs.

An executive summary should be 2-3 pages, is formatted based on APA guidelines, and includes in-text citations and a reference page including readings and resources presented this week.
